Primary Care: Mental Health Integration (MHI)

It’s normal to feel stressed, anxious, down or moody at times, but if these feelings persist, your Primary Care-Mental Health Integration (PC-MHI) team can help.

Who is PC-MHI care for?

Any Veteran in primary care has access to PC-MHI’s resources and support team.

Talk to your Primary Care Provider (PCP) if you have any of these common concerns:
  • Stress
  • Anxiety
  • Feeling down or hopeless
  • Trouble sleeping
  • Irritability

Why get PC-MHI care?

PC-MHI care is problem-focused, solution-oriented, and patient-centered.

PC-MHI treatments and recommendations focus on your goals, and what is important to you right now, to give you the best options available for your care.

PC-MHI care has proven to be effective.

  • access to high quality treatment,
  • patient engagement,
  • clinical and functional outcomes, and
  • patient satisfaction.

What can PC-MHI care do for you?

PC-MHI works with your Patient Aligned Care Team (PACT) to meet your mental health needs right in primary care.

Your PC-MHI Care Manager will help you understand your symptoms, how treatment can help, and what you can do to help. PC-MHI Care Managers also track your treatment, progress, and concerns, and coordinate any adjustments as needed.

Along with your Care Manager, PC-MHI mental health experts work with you and the rest of your PACT. These mental health experts are located in the primary care clinic, and offer tailored care focusing on your concerns.
