Adult Day Health Care

Adult Day Health Care provides for general health care and social needs of Veteran patients. In many cases, this service makes it possible for patients to continue living at home or with family. Having a safe place to bring a family member who needs help during the day makes caring for them more manageable. This service may be offered through a VA-managed program or through a community-based program that VA pays for. Veterans receive care in a safe, structured setting during the day and return to their own homes in the evening.

  • Recreational and physical exercise
  • Recreational activities
  • Health education & medication management
  • Weight and blood pressure monitoring
  • Meals
  • Care for patients with dementia
  • Support and information for family caregivers

Adult day health care is available after a consult with a VA primary care provider or social worker. Contact your Patient Aligned Care Team for more information.
