There are many ways for you and your family members
to become more informed and take an active role
in your healthcare. We want you and your family
to know about your health and how to best take care
of you. There are a variety of programs, materials,
and activities available. Programs are held on diabetes,
cardiac care, weight management, stroke, oncology,
smoking cessation, specialized support groups, and
other health topics. Tell your primary care team
if you want to sign up or if you need additional
information.
Your primary care team may also arrange for other
materials (models, audiotapes, pamphlets, etc.)
for you and your family to look at or they might
ask you to visit the Patient Education Resource
Center (PERC). The PERC is open to all veterans
and their families. The PERC has books, videos,
and Internet resources. All items are for on-site
use. Pamphlets are available on a variety of health
topics.
