All across America, the
number of homeless persons is on the rise.
Estimates of their numbers range from 250,00
to more than 3,000,000 and their numbers are
increasing by roughly 25% each year. Approximately
30% of America’s homeless population
are veterans. In response to this national
problem the Veterans Administration created
the Health Care for Homeless Veterans Grant
and Per Diem Program. This program is designed
to break the cycle of homelessness and help
veterans get reestablished in the community.
If you know a homeless veteran, encourage
them to contact one of the following VA Homeless
Staff for assistance and eligibility requirements.

Law 100-6 established the Health Care
for Homeless Veterans Program (HCHV).
Greater Cincinnati/Northern Kentucky area.
The HCHV program provides extensive outreach,
physical and psychiatric examinations,
treatment referrals and on-going case
management to homeless veterans with mental
health and chemical dependency problems.

The
HCHV staff can be the contact and advocate
between the veteran and services that,
in the past, were not accessed by the
homeless veteran.

- Transitional housing
for eligible veterans
- Psychiatric treatment:
Inpatient and Outpatient
- Alcohol/Drug Detoxification
and Rehabilitation
- Case Management services
while in transitional housing
- Training Programs with
Job Placement potential

VA's Homeless Providers
Grant and Per Diem Program is offered annually
by the Department of Veterans Affairs Health
Care for Homeless Veterans (HCHV) Programs
to fund community agencies providing services
to homeless veterans. The purpose is to promote
the development and provision of supportive
housing and/or supportive services with the
goal of helping homeless veterans achieve
residential stability, increase their skill
levels and/or income, and obtain greater self-determination.
