In order to help us serve you, your primary care team will need copies of your past medical history for conditions not treated at the Cincinnati VAMC. A Release of Information Form is used for this purpose. You can use this same form to send information to outside sources (e.g. insurance companies, employers, other physicians, etc.) There may be a charge for some services.
